Approach to Individual Services
Encouraging the consumer to participate in the goals stated in their individual service plan does promote skill acquisition, self confidence and independence.  Each item below enables Absolute Care to develop training tools and documentation books to monitor the Successful interaction between direct care staff and the consumer as well as written documentation to support the on-going active treatment of goal pursuit.

  • House Specific Training Checklist – trains staff on the policies, procedures and documentation specific to the home in which they are working.

  • Consumer Specific Training Checklist – trains staff on the policies, procedures and documentation specific to each consumer within the home.

  • Consumer Specific Documentation - books that include ISP, Medical and financial information as well as a Staff Communication Log.

Health Services Coordination
Health Service Coordination is a vital aspect of serving the “whole” consumer.  Absolute Care believes that ensuring one’s health care and the coordination of services needed to maintain one’s overall physical and mental health will enhance the quality of life that one is able to live.

  • 3 Full time Registered Nurses on staff
  • Completes Initial Medication Assessment
  • Sets up delivery of medication with pharmacy
  • Monthly Medical Quality Assurance Reviews
  • Monitors Vitals Monthly
  • Monthly Visual monitoring of the consumer
  • Monthly review of consumer’s medications
  • Monthly preparation of Medication Administration Record
  • Participation in House Meetings on a quarterly basis
  • Prepares bi-annual Health Care Plan
  • Coordinates all health care services
  • Follows up on all services received and requested
  • Attends medical appointments as requested
  • Maintains communication with all team members

Payeeship
Absolute Care provides Social Security Payeeship services free of charge to each consumer as requested.

  • Social Security checks are mailed directly to Absolute Care’s office
  • Checks are deposited into each consumer’s checking account at the beginning of the month
  • All funds needed for bills, spending money and other expenses can be accessed at Absolute Care’s office
  • Account is reconciled and maintained by Absolute Care’s payee
  • Checkbooks and all financial documentation are kept in a locked drawer in the payee’s office
  • Monthly Cost-To-Lives are completed and forwarded to the county board
  • Annual Social Security reports are completed
  • Annual re-determination information is gathered and supplied to Social Security
  • Guardians have ability to review consumer’s checkbook at their will

Quality Assurance
At Absolute Care, we pride ourselves on providing the highest quality of services to our consumers. Our Quality Assurance Department is an integral part of establishing, monitoring and ensuring the best possible care.

  • Full time Quality Assurance Coordinator visits each consumer in their home one time a month.
  • Director of Quality Assurance conducts a quality assurance review in several areas:
    • Ensuring the documentation reflects the ISP services, medical/behavioral responsibilities and recreational opportunities specific to each consumer.
    • Checking that each consumer’s home is clean, the consumer’s hygiene is good and there is ample food in the home. Confirming that staff is trained appropriately and is compliant with all necessary certifications.
  • Director of Quality Assurance meets with each consumer while in the home to discuss their concerns and get their input and feedback on the care they are receiving.
  • House manager is given a score based on the services being provided to their consumer and any score below 85% requires them to attend a meeting with the Quality Assurance Coordinator to make all necessary adjustments.
  • Any concerns regarding the health and safety of the consumer are immediately reported to the Director of Operations and CEO of the agency.
  • Program Coordinator reviews QA and submits a follow up response to the Quality Assurance Coordinator.
  • Quality Assurance reviews are tracked monthly and annually to monitor the progress of each consumer based on the services provided.
